Meds-to-Beds: On the Way to Better Health
Harris Health System’s Meds-to-Beds Program is a bedside service designed to improve health outcomes and ease the transition for patients from the hospital to home.
This program delivers discharge medications to a patient’s bedside before they leave the hospital. There is no need for the patient, a friend, or a family member to stop at the pharmacy on the way home because the pharmacist delivers the medication to the patient. The program offers private, personalized medication counseling by a pharmacist, and patients only pay their co-pay.
The Meds-to-Beds program helps patients stay on track with their medication plan, and patients receive a follow-up phone call from a pharmacist within 72 hours after leaving the hospital to check on their progress.
