- If you’re 18 years old or older, you’ll also need to sign an ImmTrac2 Adult Consent Form. It’s important to note that to maintain immunization records in the registry, you must submit the form by your 26th birthday.
- Mail your form to the Texas Department of State Health Services which is located in Austin Texas.
