Family Houston is a local agency that can take on your case and help you to navigate Houston’s healthcare system.

Family Houston is a federally qualified health center. This means they are a federally funded clinic that provides low-cost medical treatment to uninsured and indigent populations in Houston. In addition to these services, they can also help you with connecting to programs that provide medical, food benefits, dental, and other government assistance programs.

Harris Health Case Managers and Social Workers

Harris Health System is the healthcare network that issues the Gold Card. If you have a Gold Card you can receive help navigating the Harris Health System via their in-house case managers and social workers. 

Whether you are receiving care at Ben Taub hospital or one of the Harris Health outpatient clinics, there are case managers who can help you to solve problems related to social, financial, and psychological issues. 

Harris Center (Mental Health and IDD Case Management)

Harris Center is the mental health authority of Harris County. If you do not recognize the name Harris Center, you may remember them as MHMRA Harris because that was their old name. 

If you, a family member, or a client need mental health or IDD help, Harris Center can assist you. 

They offer inpatient case managers at their outpatient clinics, crisis case managers via their MCOT or Mobile Crisis Outreach Team, 24-hour crisis counselors on their helpline, and Care Coordinators who work with IDD clients who have cognitive delays. Whether you are needing short or long-term mental health or IDD case management, Harris Health can help you.

Northwest Assistance Ministries

NAM offers case management and a number of other services to its clients. 

NW Ministries Offers The Following Services:

  • The Interfaith Homeless Network (IHN) assists homeless families with household items like cleaning items, move-in kits, limited financial assistance, or transportation expenses.

Changes Happens is a nonprofit that is located in 3rd Ward Houston, TX that provides case management services to children and women. 

The specific populations that Change Happens serves are: 

  • Children up to age 20 who receive Medicaid
  • Pregnant women who receive Medicaid  who have health problems or at risk of developing health problems

This City of Houston Juvenile program provides case management to students who appear in the court system due to citations. 

The Juvenile Case Manager program focuses on students in middle and high school who attend a campus within Houston city limits. The services provided to students are:

The Montrose Center provides supportive services to the LGBTQ community in Houston. 

Those supportive services include case management. 

The populations that the Montrose Center serves are:

  • Survivors of domestic violence, sexual assault, or a hate crime
  • LGBTQ adults 60 and older

City of Houston Health Department also offers case management to individuals who have a diagnosis of HIV, AIDS, another sexually transmitted disease.

Medicare Advantage Care Coordinators

If you have a Medicare Advantage plan, there is a good chance that you have an assigned case manager. In the insurance industry, case managers are sometimes referred to as Care Coordinators.

Editor’s Note: If you have a Medicare Advantage plan and a chronic condition like diabetes, heart disease, or high blood pressure the is a strong likelihood that there is a Care Coordinator assigned to your case.

Their role is to help you to get the most out of your Medicare Advantage plan. This means helping you connect to doctors and specialists who can treat you, informing you of medical equipment that your insurance will pay for, and connecting you to physical, occupational, and speech therapists when you need them.

Essentially any question you have about your Medicare Advantage coverage and what benefits you are entitled to should be directed to your Care Coordinator.

If you were hospitalized, your discharge planners would help you navigate the healthcare system by providing information about post-hospital care options.

It’s important that you clear up any questions that you have regarding your discharge plans with the hospital case manager. Once you are discharged you are essentially on your own…

Until then, it’s the discharge planner’s job to ensure that you have a safe discharge plan and help you with any barriers you anticipate on discharge.

Take Your Issue To Houston City Council Members

This is a day center for survivors of human trafficking who are either forced into sexual or labor exploitation.

Services are offered to survivors of human trafficking in a safe, secure office building.  This location was strategically selected to deter pimps from pursuing survivors and to serve as a more discreet location for anyone seeking help.

Bay Area Turning Point (Homeless Shelter Clear Lake, TX)

Bay Area Turning Point offers shelter to men, women, and children who are the victims of domestic or sexual assault.

Walk-ins are welcome here on Monday, Tuesday, and  Friday from 8:30 am-5:30 pm.  On Wednesdays and Thursdays, walk-in hours are 8:30 am-8:30 pm.

The following services are offered to anyone who has experienced sexual assault or family violence. You do not have to be staying at the shelter to receive these services:

  • Assistance with filing protective orders
  • Information and referral
  • Education and awareness
  • Violence against women support groups

Bristow Center –  PATH Program (Homeless Case Management)

The PATH Program is a day center for homeless individuals with mental health diagnoses. PATH is a part of Harris Center (formerly MHMRA of Harris County).

PATH is open Monday – Friday from 7 am to 4 pm, however, this is a first-come, first-serve process. 

You are encouraged to arrive at PATH around 6:30 am for a better chance of being seen. PATH typically takes 3 – 5 new clients per day which demonstrate how important it is to arrive early.

PATH Program offers the following services:

  • Linkage to Psychiatric Services.
  • A place to shower.
  • Case Management services
  • Lunch 
  • Temporary ID

The PATH Program is located in the Midtown neighborhood. The closest major intersection is Caroline and McGowen. The address is 2627 Caroline St, Houston, TX 77004.

Compass (Houston Homeless Case Management)

Compass offers case management services and helps to obtain Harris Health/Gold Card to homeless men/women in Houston.

This is NOT a homeless shelter.

Compass is open Monday through Friday from 9 am to 2 pm.

Services (case management and Gold Card help) are offered on a first-come, first-serve basis. Compass only sees the first 10 clients per day, therefore, you are advised to arrive as early as possible. Possibly 7 am?

Below is the weekly schedule for the day center’s services:

  • Gold Card assistance occurs Monday – Friday.  First 10 people, so arrive early.
  • Case Management assistance occurs Monday – Thursday. Walk-ins for Case Management is not offered on Fridays.

The following resources are offered here:

Case management, help to obtain a Gold Card, vocational counseling (JobPoints Project), clothing and haircut vouchers, reading glasses, telephone use, community voice-mail, and mail, offers bus tickets for the Project Access homeless bus.

The day center is located in downtown Houston. It is housed in the same building as The Beacon. The address is 1212 Prairie St, Houston, TX 77002.

The Beacon (Homeless Case Management Houston) 

The Beacon is a day program that offers a number of resources to homeless men and women in Houston.

This is NOT an overnight shelter.

The shelter is not open daily, therefore, it’s best to either call them at 713.220.9737 or email them at info@beaconhomeless.org for hours of operation.

Below is a brief summary of operational hours:

  • The Day Center is open Monday, Thursday, Friday from 7 am to 2 pm.
  • Beacon Law is open Monday and Friday. Doors open at 8 am

The shelter can only accommodate a limited number of clients which is why it’s important to arrive at least an hour prior to the center opening.

The Beacon offers many services to clients, here are just a few:

  • At the Day Center services like a hot lunch, access to showers and laundry services are offered. The services are offered Monday, Thursday, and Friday. The Day Center is open from 7 am to 2 pm.
  • Bridgid’s Hope is a program for women 35 and older who are recently released from prison. The goal of the program is to help women transition from the criminal justice system to self-sufficiency. Linkage to housing, a mentor, and case management are provided to clients in this program.  For eligibility requirements visit the Beacon website.   • Bacon Law provides free legal assistance to clients. Some of those services include:
    • Help obtaining IDs, resolving traffic warrants, and birth certificate problems.
    • Assistance with Social Security, VA Benefits, and elder law.
    • Housing Law regarding disputes between you and your landlord or foreclosure/title disputes.
    • Employment/Labor Law regarding criminal history corrections, wills, and consumer law.

For legal assistance, you must visit the Beacon on Mondays and Fridays and arrive at 8 am. Preferably sooner. If you are staying at a Houston area shelter your case manager can provide a referral. For more information call the Beacon Law at 713.220.9780.

The shelter is partnered with The Way Home Program. This is a housing program that helps clients gain access to permanent housing.  Beacon Houston is one of the few local sites that offer Coordinated Access.  The Coordinated Access assessment is used to determine eligibility into The Way Home Program.  For more information on the housing program call the Beacon at 713-220-9717.
